This is a stage III randomized test to evaluate whether sertraline when included in conventional amphotericin-based therapy for cryptococcal meningitis, will lead to improved survival. Cryptococcal meningitis diagnosis will be made through CSF cryptococcal antigen at time of back puncture with confirmation by CSF culture. A non-randomized phase I dose-escalation study will first be conducted to aid optimize application for a larger randomized stage II study. Stage III Design: Subjects will be randomized to conventional induction therapy with covered up sugar pill or sertraline at 400mg/day. We will use a permutated block randomization in a 1:1 allotment.
